An early Bitcoin adopter had backed up their wallet private keys to a LastPass account. When LastPass suffered a security breach in 2013, the user changed their master password and in doing so lost access to their account due to a recovery email that was no longer valid. The wallet keys stored inside LastPass were permanently inaccessible.
