A Trezor forum user reported in early 2022 that they had transferred substantial Bitcoin from a Ledger to a Trezor Safe 3, set up a hidden wallet with a passphrase, confirmed the seed phrase during setup, but found themselves locked out months later. On reconnection the same passphrase produced an empty wallet with a different receive address. Investigation revealed the passphrase had likely been entered with a character variation—a space, case difference, or special character—that was undetectable to the user.
