A user who had set a complex BIP39 passphrase in 2019 attempted to access their hardware wallet in July 2022 to move funds off exchanges following the Celsius collapse. The passphrase had been stored in a password manager that was itself inaccessible after the user forgot the master password. The user retained the seed phrase and the hardware device but could not reconstruct the passphrase. The compounding of two separate access failures—passphrase manager and wallet passphrase—created a recovery scenario with no straightforward path.
