Multisig quorum failure — 4-of-7 multisig (2021)

A DeFi protocol's 4-of-7 multisig treasury was paralysed in May 2021 when two key holders — both anonymous pseudonymous developers — disappeared after a governance dispute. With only five key holders identifiable, and no contact from two, the treasury could not be moved. The remaining key holders attempted to pass a governance vote to migrate to a new wallet but lacked the quorum to execute.
